Do we legally need a staff handbook?

The handbook as a single document is not itself a legal requirement, but several things practices sensibly keep in it are required in some form, such as disciplinary and grievance information. The honest answer is that the handbook is how you prove the sensible things were written down.

How often should the handbook change?

Review it once a year and whenever the law or your practice changes materially. The failure mode is not reviewing too rarely, it is making changes and being unable to show who saw which version. Version control and acknowledgements matter more than frequency.
